长期胃肠外营养早产儿游离肉碱及相关代谢的改变 被引量:7

摘要 目的 了解长期胃肠外营养早产儿游离肉碱水平的变化和相关代谢的改变.方法 对新生儿科重症监护室胎龄为28~32周的65例需全胃肠外营养时间≤3 d(对照组)或>7 d(观察组)早产儿进行分析,测定并比较两组生后第7、14天的血游离肉碱、血脂及血糖水平.结果 与对照组比较,第14天时观察组游离肉碱水平明显下降(t=2.42,P=0.02),三酰甘油明显升高(t=2.21,P=0.03),低密度脂蛋白胆固醇水平明显升高(t=2.67,P=0.01),血糖水平明显偏低(t=3.51,P=0.001).长期胃肠外营养早产儿血游离肉碱水平与三酰甘油、低密度脂蛋白胆固醇水平呈负相关(r=-0.38,P=0.03;r=-0.43,P=0.02),与血糖水平呈正相关(r=0.56,P=0.001).结论 长期胃肠外营养早产儿游离肉碱水平会下降,导致血糖及血脂代谢紊乱,应定期检测游离肉碱水平及监测相关代谢指标.
