归泽苓翘方治疗成人髋关节滑膜炎的回顾性研究

Retrospective Study on Adult Synovitis of Hip by Therapy of GuiZheLingQiao Decoction

目的：初步探讨成人髋关节滑膜炎与成人髋臼发育不良的相关性及中药内服外敷治疗的临床疗效。方法：回顾性分析2011年1月～2012年5月在我院髋一科门诊及住院收治的33例成人髋关节滑膜炎的病例：男18例、女15例；年龄16～69岁，平均40岁；单侧9例、双侧24例，共57髋。所有患者均给予内服利水消肿中药12周，同时外敷我院自制的骨炎膏3～4周。所有病人行DR、MRI检查。结果：1．轻度髋臼发育不良的23例，中度8例，重度1例；2．所有髋关节内均存在不同程度积液；3．所有患者均获得随访，通过电话联系或病人定期复查的方式进行随访，随访2．5～15m，平均8．5m。32例中，19例治愈（59．37％），12例好转（37．5％），1例未愈（3．13％）。结论：1．成人髋臼发育不良与成人髋关节滑膜炎成正相关；2．MRI可以作为诊断该病的必检项目；3．中药内服外敷治疗成人髋关节滑膜炎有良好的临床效果。

Objective：To explore the correlation between adult synovitis of the hip and adult acetabular dysplasia, and the clinical effects of combination with oral administration and external application of Chinese medicinal herbs. Methods.. All 33 cases of synovitis of the hip were analyzed retrospectively, including 18 males and 15 females. The age was from 16 to 69, and mean age was 40. Nine patients were unilateral, and 24 patients were bilateral. All patients were received traditional Chinese medicine inducing diuresis to alleviate edema for 12 weeks and external application of Guyan plaster for 3 to 4 weeks, examined by DR and MRI. Results：All 24 cases were mild acetabular dysplasia, 8 cases were moderate, and 1 case was severe. There was hydrops of hip joint in all the cases detected by MRI. They were followed up for 2.5 to 15 months, 8.5 months in averagely. There were 19 cases （57.6%） cured, 13 cases （39.43~） improved, and 1 case did not healed. Conclusion：There is positive correlation between adult synovitis of the hip and adult acetabular dysplasia. MRI is necessary for the diagnosis of adult transient synovitis of the hip. Combination with oral administration and external application of Chinese herbs is effective, safe, convenient, and economy, and it is worthy for promotion.
