虚拟现实技术在颈内-后交通动脉动脉瘤个体化治疗中的应用 被引量:5

摘要 目的探讨Destroscope虚拟现实系统在颈内-后交通动脉动脉瘤个体化治疗中的应用价值。方法回顾性分析首都医科大学附属复兴医院2011年5月至2013年5月应用Destroscope虚拟现实系统做术前计划治疗的颈内-后交通动脉动脉瘤病例18例,术前均将其CTA原始数据等以DICOM格式输入Destroscope虚拟现实系统中进行三维重建、分割,观察并模拟操作设计个体化方案,指导治疗。结果18例颈内-后交通动脉动脉瘤病例均成功地实现了颅骨及颅内血管的三维立体虚拟现实影像的重建。其中16例与CTA报告结果一致,2例CTA报告可疑,在虚拟现实影像上清晰显示动脉瘤。18例患者均成功进行虚拟现实的手术模拟与手术设计,所得3D影像与手术所见几乎一致。结论Destroscope虚拟现实系统能够清楚地显露动脉瘤大小、位置,并可量化与周围结构的关系,有助于动脉瘤的诊断及最佳治疗方案的选择,提高治疗效果。 Objective To evaluate the application of virtual reality technique in diagnoseand individualized therapeutic planning of internal carotid - posterior communicating artery aneurysm. Methods Retrospective analysis 18 cases of internal carotid - posterior communicating artery aneurysm from May 2011 to May 2013, whose surgery programs were performed by Destroscope virtual reality system. The preoperative primal CTA data was saved in DICOM format and transferred into the VR computer for restitution, dissection, and surgery simulation to direct therapy. Results Virtual reality three -dimensional images of the 18 internal carotid - posterior communicating artery aneurysm cases were successfully achieved, among the restdts, 16 cases supported the CTA results , 2 cases who were suspicious in CTA results, were finally diagnosed aneurysms on the virtual reality images . Surgical simulation and surgery program of virtual reality of the 18 patients were successfully performed. The 3D images obtained from virtual reality near to the real surgery. Conclusion Destroscope virtual reality system can dearly reveal the size, location of aneurysm, and quantify the relationships between aneurysm and surrounding structures, which is helpful to diagnose and select the best individualized therapeutic planning to improve outcome.
