肢体神经鞘瘤的超声诊断及误诊分析 被引量:4

摘要 目的 探讨高频超声诊断肢体神经鞘瘤的临床价值.方法 回顾性分析我院2008年1月~2013年7月18例经手术和病理证实的肢体神经鞘瘤声像图特征,并分析其误诊原因.结果 18例肢体神经鞘瘤中超声检出率100%,其中低回声型13例,混合回声型3例,无回声型2例.超声诊断符合率44.4%(8/18),其中2例误诊为神经纤维瘤,2例误诊为肌纤维瘤,1例误诊为血肿,1例误诊为腱鞘囊肿,4例性质待定.结论 肢体神经鞘瘤声像图有其特征性,但需与神经纤维瘤、肌纤维瘤、血肿、腱鞘囊肿等疾病鉴别,以提高超声诊断准确率. Objective To explore the high - frequency ultrasound clinical value on limb schwannoma. Meth- otis The sonographic features and the causes of misdiagnosis were analyzed retrospectively on 18 limb schwannoma confirmed by surgery and pathology in our hospital from January 2008 to July 2013. Results The ultrasound detec- tion rate in 18 limb schwannomas was 100% and the ultrasonographic features included weak echoic reflection 13 cases, tangled echoic reflection 3 cases, fluid echoic reflection 2 cases. The coincidence rate was 44.4 %, 2 cases misdiagnosed as neurofibromatosis, 2 cases misdiagnosed as myofibroma, 1 case misdiagnosed as hematoma, 1 case misdiagnosed as ganglion cyst and 4 cases undetermined nature. Conclusion Limb sehwannomas have their characteristic sonographic, but need to identify with neurofibromatosis, myofibroma, hematoma, ganglion cysts and other diseases, in order to improve the diagnostic accuracy of ultrasound.
