利用去囊导管下咽部给氧快速纠正无痛胃镜肥胖患者血氧饱和度的研究 被引量:2

Effets of Catheter without Sack Placed around Hypopharynx on Affording Oxygen for Correcting Hypoxemia of Obese Patients during Sedative Gastroscopy

摘要 目的探讨利用去囊后的气管导管(简称去囊导管)下咽部给氧,快速纠正无痛胃镜检查中肥胖患者血氧饱和度的可能性。方法选择无痛胃镜肥胖患者1274例,随机分为鼻导管组(A组)和下咽部导管组(B组),当无痛麻醉过程中血氧饱和度下降10%以上时,A组采取加大鼻导管氧流量、抬高下颌等方式纠正低氧状态,B组通过去囊导管下咽部供氧纠正低氧状态。结果 A组和B组患者出现血氧饱和度下降10%以上者分别为617例(96.86%)和598例(93.86%),两组无明显差异(P>0.05)。A组中303例(49.11%)采取加大鼻导管氧流量、抬高下颌等方式改善通气量后完成检查,B组中577例(96.48%)插入去囊导管后完成检查,明显高于A组(P<0.05);A组麻醉后的最低血氧饱和度为(83.12±1.65)%,与B组无明显差异(84.92±1.78)%,其中A组采取纠正措施后的SpO2升至(90.57±1.74)%,明显低于B组(96.41±1.56)%(P<0.05)。结论利用去囊后的气管导管下咽部给氧可快速纠正无痛胃镜肥胖患者血氧饱和度的下降,提高检查成功率。 Objective In order to improve SpO2 of obese patients during sedative gastroscopy, we investigated the effect of tracheal catheter without sack (catheter without sack) placed ai'ound hypopharynx on affording oxygen. Meth- ods 1274 obese patients with sedative gastroseopy were randomly divided into group A (nasal catheter group) and group B(traeheal catheter group). When SpO2 decreased more than 10%, the hypoxemia of group A was improved by increasing the oxygen flow and raising submaxilla,but catheter without sack was placed around hypopharynx in group B. Results There are no differences between group A (617,96.86//00) and group B (598,93.86o//00) whose SpO2 de- creased more than 10% during anesthesia(P 〉 0.05). 303 patients(49.11/00) of group A accomplished sedative gas- troscopy by taking some correcting measures,which is lower than group B(577,96.48% ,P 0.05). The lowest SpO2 of group A after anesthesia was (83.12 Jr 1.65)%, which are no differences with group B (84.92 ! 1.78)%. The SpO2 of patients who suffered from hypoxemia of group A was increased to(90.57 --k-1.74)% by correcting method,which is significantly lower than group B (96.41 --4-_ 1.56) %0 (P 0.05). Conclusion The hypoxemia of obese patients during sedative gastroscopy can be improved by placing tracheal catheter without sack around hypopharynx, which increases the achievement ratio of sedative gastroseopy.
