摘要
目的研究关节镜下微骨折术联合玻璃酸钠注射治疗膝骨关节炎软骨缺损的治疗效果。方法收集2011年4月至2012年9月在合肥市第三人民医院行关节镜手术治疗的31例膝骨关节炎软骨缺损患者。采用关节镜下微骨折术治疗,术后立即注射玻璃酸钠20 mg,术后第1周、第2周均注射玻璃酸钠20 mg。术后第8周、第16周膝关节功能Lysholm得分采用样本均数t检验,并采用Tegner运动评级进行膝关节功能评价。结果术后第8、16周:手术前后膝关节功能Lysholm得分均有差异(P<0.05);术后第8周Tegner运动评级,优11例,良16例,差4例,优良率为87.1%。术后第16周Tegner运动评级,优11例,良15例,差5例,优良率为83.9%。结论微骨折术联合玻璃酸钠注射治疗膝骨关节炎软骨缺损取得满意的治疗效果。该方法具有操作简便、创伤小、恢复快、并发症少的特点,可明显改善患者的关节功能并减轻疼痛症状。
Objective To investigate the effect of arthroscopic mierofracture technique combined with sodium hyaluronate injection in the treatment of cartilage defects in knee osteoarthritis. Methods 31 cases of patients with the cartilage defects of knee osteoarthritis were treated with arthroscopic operation in the Third People's Hospital from April 2011 to September 2012. All the patients were treated with artbroscopic mierofraeture technique, sodium hyaluronate were injected into the articular cavi- ty after the operation immediately, and also in 1 week and 2 week after the operation. Lysholm score were compared between 8 week and 16 week using test,the knee joint movement function were evaluated by Tegner rating. Results The Lysholm score were significant different before and after the operation ( P 〈 0.05 ). The Tegner rating in 8 week : excellent results was 11 ca- ses, good was 16 cases, poor was 4 cases,the total rate of excellent and good clinical results was 87.1%. According to Tegner rating, there were excellent in 11 cases, good in 15 cases, poor in 5 cases,in 16 week. And the total rate of excellent and good was 83.9%. Conclusion It showed to us that arthroscopie mierofractnre technique combined with sodium hyaluronate injec- tion achieve a satisfactory therapeutic effect. This technique was liable to operate with little invasion, and patients can recovery quickly with small complications. And it can significantly improve joint function and reduce pain symptoms of the patients.
