摘要
目的:探讨阿立哌唑与氯氮平治疗精神分裂症阳性症状疗效的差异。方法:计算机检索CENTRAL、PubMed、EMBASE、中国生物医学文献数据库(CBM)、中国期刊全文数据库(CNKI)、中文科技期刊全文数据库(VIP)等,检索时间从各数据库建库至2013年9月20日,同时辅助其它检索。应用Meta分析方法对纳入的44项阿立哌唑与氯氮平治疗精神分裂症的随机对照研究进行再分析,重新评价其疗效和生活质量。结果:①阿立哌唑治疗精神分裂症阳性症状前后的自身对照,差异有统计学意义[MD=-12.46,95%CI(-12.46,-11.43),P<0.00001]。②阿立哌唑与氯氮平在改善精神分裂症患者阳性症状的组间比较,差异无统计学意义[MD=0.30,95%CI(-0.35,0.94),P=0.37]。③阿立哌唑与氯氮平治疗精神分裂症的有效率无统计学差异[OR=0.96,95%CI(0.76,1.20),P=0.70]。④在改善生活质量方面,阿立哌唑组优于氯氮平组,且差异具有统计学意义[MD=2.60,95%CI(2.24,2.96)P<0.00001]。结论:阿立哌唑与氯氮平在治疗精神分裂症阳性症状的疗效无统计学差异,但阿立哌唑能更好地改善患者生活质量。
Objective： To study the difference of efficacy between aripiprazole and clozapine in the treatment of schizophrenia with positive symptoms. Methods： We systematically searched CENTRAL, PubMed, EMBASE, Chinese Biomedical Literature Database （CBM）, China National Knowledge Infrastructure （CNKI） , VIP Database from inception through September 20, 2013. 44 studies that compared effects difference in treatment of positive symptoms between aripiprazole and clozapine were analyzed by meta-analysis to re-e- valuate the therapeutic effects of the two drugs. Results： （1）Pre-treatment and post-treatment for aripiprazole was compared, the result showed that aripiprazole has good effect on positive symptoms [ MD =-12. 46, 95% CI （-12.46, -11.43）, P 〈 0. 00001 ]. （2）Compared post-treatment between aripiprazol and clozapine for positive symptoms of schizophrenia, the difference was statistically insignificant [ MD = 0. 30, 95% CI （ -0. 35,0.94 ） , P = 0. 37 ]. （3）The efficiency was similar for aripiprazole and elozapine, there is no statistically signif- icant [OR：0.96, 95%CI （0.76, 1.20）, P =0.70]. （4） Aripiprazole can statistically significant improve the quality of life [ MD = 2.60, 95% CI （2. 24,2. 96） , P 〈0. 00001 ]. Conclusion： Aripiprazole and clozapine was no significant difference in the treatment of positive symptoms of schizophrenia, however, aripiprazole was better for quality of life.
