干预前后我院骨科Ⅰ类切口手术预防使用抗菌药物比较

Prophylactic use of antibiotics before and after pharmacists＇ intervention on type Ⅰ incision surgery in orthopedic department of our hospital

目的 通过对比分析干预前、后我院骨科Ⅰ类切口手术围手术期预防用药情况,探讨临床药师干预的有效性.方法 回顾性调查骨科201 2年第一季度和2013年第一季度Ⅰ类切口手术病例,对每月Ⅰ类切口手术预防用药比例、预防用药品种选择合理率、预防用药时机合理率、平均预防用药时间等具体指标进行统计,分析实施干预前后抗菌药物使用情况.结果 干预后骨科Ⅰ类切口手术预防使用抗菌药物的比率显著下降,由干预前的90.34％降至干预后的54.69％;平均用药天数由干预前的2.24天下降到干预后的1.55天,24 h内停药百分率由干预前的21.40％升至干预后的63.43％.结论 临床药师通过参与抗菌药物临床应用管理,使得骨外科Ⅰ类切口手术不合理使用抗菌药物的现象明显改善,干预措施对规范使用抗菌药物有明显的促进作用.

Objective To explore the effectiveness of clinical pharmacists＇ intervention comparatively analyzing the prophylactic use of antimicrobial agents before and after the intervention on type I incision surgery during perioperation period in the orthopedic department of our hospital. Methods The cases undergoing type I incision surgery who were selected from the orthopedic department of our hospital in the first quarters of 2012 and 2013 were retrospectively investigated. The proportions, rational rates of drug selecting and timing, mean times of prophylactic drug use for type I incision surgery in each month were statistically analyzed. Results After the invention, the proportion of prophylactic drug use dropped from 90.34% to 54.69%, the mean time shortened to 1.55 days from 2.24 days, drug withdrawal rate within 24 hours increased from 21.40% to 63.43%. Conclusions If the clinical pharmacists take part in the management of antibiotics use, the irrational use of antimicrobial drugs for type I incision surgery in orthopedic department will be lessened. Intervention measures play a significant role in regulating the use of antimicrobial drugs.
