单侧及双侧椎间融合内固定治疗腰椎间盘突出症的临床疗效评价 被引量:5

CLINICAL EVALUATION OF UNILATERAL AND BILATERAL INTERBODY FUSION COMBINED WITH INTERNAL FIXATION IN TREATING LUMBAR DISC HERNIATION

摘要 目的研究单侧及双侧椎间融合内固定治疗腰椎间盘突出症的临床疗效。方法将2011年1月—2012年12月我院骨科收治的腰椎间盘突出症患者纳入研究,随机分为给予单侧椎间融合内固定治疗的观察组和给予双侧椎间融合内固定治疗的对照组,观察手术相关指标及近期、远期疗效指标。结果①观察组患者的手术时间、术中出血量、术后引流量以及肛门排气时间、下床活动时间均低于对照组;②术后,二组患者的椎间隙高度、椎间隙曲度、融合率以及JOA评分、ODI评分、SF-30评分的差异均无统计学意义。结论单侧椎间融合内固定治疗有助于减小手术创伤、促进术后恢复,且取得与双侧手术相当的近远期疗效。 Objective To study the clinical effects of unilateral and bilateral interbody fusion combined with internal fixation in treating lumbar disc herniation. Methods Lumbar disc herniation patients in the department of orthopedics in author's hospital from January 2011 to December 2012 were enrolled and randomly divided into observation group given unilateral interbody fusion combined with internal fixation and control group given bilateral interbody fusion combined with internal fixation. Then operation related index, short- term effect and longterm effect were observed. Results Operation time, intraoperative bleeding volume, postoperative drainage volume and anal exhaust time, ambulation time of observation group were lower than those of control group. There was no significant difference between two groups in intervertebral space height, intervertebral space curvature, fusion rate, JOA score, ODI score, SF- 30 score. Conclusion Unilateral interbody fusion combined with internal fixation can reduce operation trauma and promote postoperative recovery, and achieve the equal shortterm effect and long- term effect with bilateral operation.
