摘要
目的观察丹参川芎嗪注射液联合胰激肽原酶治疗早期糖尿病肾病(DN)的临床疗效。方法将76例早期DN患者随机分为2组,对照组37例予胰激肽原酶肌肉注射治疗,治疗组39例在对照组基础上加用丹参川芎嗪注射液静脉滴注治疗。2组均治疗2周。测定2组治疗前后血肌酐(Cr)、尿素氮(BUN)、肾小球滤过率(GFR)、胆固醇(TC)、甘油三酯(TG)及24 h尿蛋白定量的变化。结果对照组Cr、TC、TG及24 h尿蛋白定量均较治疗前明显下降(P<0.05),BUN及GFR变化比较差异均无统计学意义(P>0.05)。治疗组治疗后Cr、BUN、GFR、TC、TG及24 h尿蛋白定量改变与本组治疗前比较差异均有统计学意义(P<0.01),且与对照组治疗后比较差异有统计学意义(P<0.05)。结论丹参川芎嗪注射液联合胰激肽原酶治疗早期DN,可降低患者Cr、BUN、24 h尿蛋白定量及血脂水平,升高GFR,改善肾功能。
Objective To observe the effect of Danshen Chuanxiongqin injection combined with pancreatic kininogenase on the treatment of early diabetic nephropathy ( DN) patients .Methods 76 cases of early diabetic ne-phropathy patients were randomly divided into treatment group and control group .Patients in two groups were treated with pancreatic kallikrein 40 U,intramuscular injection,once a day.Patients in the treatment group were added with Danshen Chuanxiongqin injection 10 mL plus normal saline 250 mL,1 times/day,intravenous infusion ,2 weeks.The changes of serum creatinine (Cr),blood urea nitrogen (BUN),glomerular filtration rate (GFR),blood lipid (TC, TG) and 24 h urinary protein before and after treatment were measured .Results In the control group ,Cr,TC,TG and 24 h urine total protein after treatment were decreased significantly as compared with those before treatment ( P0.05).The treatment group Cr,TC,TG,24 h urine total protein,BUN and GFR after treatment were decreased signifi-cantly as compared with those before treatment in treatment group (P〈0.01).Compared with the control group ,the difference was statistically significant (P〈0.05).Conclusion Danshen Chuanxiongqin injection combined with pancreatic kininogenase for treatment of early diabetic nephropathy can significantly decrease the Cr ,BUN,24 h urine protein content and lipid levels ,increase GFR and improve renal function .
