摘要 目的研究Leeds—Keio人工韧带在膝关节韧带损伤修复中的有效性和安全性。方法将2011年1月~2012年12月在长海医院骨科关节病区就诊的膝关节韧带损伤患者纳入研究,采用随机数表法分为Leeds—Keio组和自体移植物组,于术前和术后3、6、9、12、15、18个月时进行Lysholm、Tenger膝关节运动水平评分。结果①术前,两组患者Lysholm膝关节运动水平评分[(43.1±6.3)分比(42.8±7.1)分]、Tenger膝关节运动水平评分[(2.18±0.55)分比(2.21±0.49)分1比较,差异无统计学意义(t=1.039、0.982,P〉0.05);②术后两组患者Lysholm—Tenger膝关节运动水平评分的差异有统计学意义(P〈0.05);③Leeds—Keio组:术后3个月起,Lyshohn膝关节运动水平评分[(69.9±8.4)分比(43.1±63)分]、Tenger膝关节运动水平评分[(4.89±0.67)分比(2.18±0.55)分]均高于术前,差异有统计学意义(t=5.983、7.763,P〈0.05),且术后15个月时评分最高;④自体移植物组:术后6个月起,Lysholm膝关节运动水平评分[(61.4±8.9)分比(42.8±7.1)分]、Tenger膝关节运动水平评分[(4.51±0.62)分比(2.21±0.49)分]均高于术前,差异有统计学意义(t=5.983、7.763,P〈0.05)。结论Leeds—Keio人工韧带能够更为有效地改善患者的膝关节功能.且在术后15个月时达到最佳的治疗效果。 Objective To study the effectiveness and safety of Leeds-Keio artificial ligament in the repairment of knee ligament injury. Methods Patients withknee ligament injury in Orthopedics Department of Changhai Hospital from Jan- uary 2011 to December 2012 were enrolled and divided into Leeds-Keio group and autograft group by random number table method. Then before operation and 3, 6, 9, 12, 15, 18 months after operation, Lysholm knee movement level score and Tenger knee movement level score were conducted. Results ①Before operation, there were no statistically signifi- cant differences between two groups of Lysholm score and Tenger score [(43.1±6.3) scores vs (42.8±7.1) scores, t = 1.039; (2.18±0.55) scores vs (2.21±0.49) scores, t = 0.982, P 〉 0.05]; ②after operation, there were statistically significant dif- ferences between two groups of Lysholm-Tenger score (P 〈 0.05); ③)Leeds-Keio group: Lysholm score and Tenger score were higher than those before operation from 3 months after operation, the difference had statistically significant differ- ences [(69.9±8.4) scores vs (43.1±6.3) scores, t = 5.983; (4.89±0.67) scores vs (2.18±0.55) scores, t = 7.763, P 〈 0.05], the score of 15 months after operation was the highest; ④autograft group: Lysholm score and Tenger score were higher than those before operation from 6 months after operation, the difference had statistically significant differences [(61.4± 8.9) scores vs (42.8±7.1) scores, t = 5.983; (4.51±0.62) scores vs (2.21±0.49) scores, t = 7.763, P 〈 0.05]. Conclusion l,eeds-Keio artificial ligament can effectively improve the knee joint function and achieve the best therapeutic effect after operation for 15 months.
