幽门螺杆菌感染和p53表达与胃癌及胃癌前病变关系的临床研究

Clinical study of the relationship between Helicobacter pylori infection and p53 expression in gastric cancer and precancerous lesion

目的探讨幽门螺杆菌(Hp)感染、p53表达与胃癌及胃癌前病变的关系。方法收集聊城市人民医院疑似胃癌患者手术切除的胃黏膜标本及胃镜活检采取的胃黏膜标本140例,分为胃良性病变(BGL)组29例、胃癌前病变(GPL)组47例和胃癌(GC)组64例,采用Warthin-Starry染色法和快速尿素酶试验观察不同胃黏膜组织中Hp的感染情况,采用免疫组化S-P法检测p53。结果 GC组和GPL组Hp感染率、p53阳性表达率高于BGL组,差异有统计学意义(P<0.05)。3组Hp阳性患者p53阳性表达率均高于Hp阴性患者,差异有统计学意义(P<0.05)。结论 Hp感染与胃癌的发生密切相关,与细胞凋亡调控蛋白p53可能存在相互协同的关系。

Objective To investigate the relationship between Helicobacter pylori （Hp）infection and p53 expression in gastric cancer and precancerous lesion.Methods 140 cases of gastric resected specimens and endoscopic mucosal biop- sy specimens from Liaocheng people＇s hospital were collected and divided into benign gastric lesion（BGL）group（29 cas- es）,gastric precancerous lesion（GPL）group（47 cases）and gastric cancer（GC） group（64 cases）.Hp in gastric mucosa was iden- tified by W-S staining and rapid urease test.Expression of apoptosis-related protein p53 in the gastric tissue was deter- mined by immunohistochemistry.Results The rate of Hp infection and the positive expression ratein of p53 in GC group and GPL group were higher than that in BGL group,with statistical difference （P〈0.05）.The positive expression rate of p53 of Hp infection patient in three groups were higher than that of Hp - negative patient,with statistical difference（P〈 0.05）.Conclusion Hp infection is closely associated with gastric cancer,there may be mutual synergistic relationship with apoptosis regulatory protein p53.
