摘要
目的总结真菌性食管炎患者的临床及内镜特征、发病因素。方法回顾性分析118例经胃镜和刷检涂片检查确诊为真菌性食管炎患者的临床资料。结果真菌性食管炎症状多样,不全表现为食管症状;内镜Kodsi分级Ⅰ级30例,Ⅱ级57例,Ⅲ级23例,Ⅳ级8例,感染部位以全段为主,其次为中下段;抗真菌治疗2周症状消失95例,复查胃镜70例,62例食管黏膜恢复正常。结论积极治疗原发疾病,减少医源性发病因素是防治真菌性食管炎的关键,抗真菌治疗效果良好。
Objective To summarize the clinical and endoscopic features,risk factors of the patients with fungal esophagitis. Methods The clinical data of 118 patients with fungal esophagitis diagnosed by gastroscope and brush in- spection smears were retrospectively analyzed. Results There were a variety of symptoms on fungal esophagitis rather than all was esophageal symptoms.Kodsi class according to endoscopic manifestation:class Ⅰ in 30 cases,class Ⅱ in 57 cases,classⅢ in 23 cases and class Ⅳ in 8 cases,the infection in given priority to with entire section,followed by the middle and lower segment.The clinical symptoms were disappeared in 95 cases after antifungal treatment for 2 weeks. There were 70 cases for reexamination gastroscopy among 62 cases of normal esophageal mucosa. Conclusion Active treatment the primary disease and reduce iatrogenic disease factors is the key to the prevention and treatment of fungal esophagitis.h is a good effect with the treatment of antifungal.
