膀胱嗜铬细胞瘤病理检查误诊为膀胱癌

Pheochromocytoma of Bladder Misdiagnosed as Cancer of Bladder by Pathological Examination

目的探讨膀胱嗜铬细胞瘤的诊断及治疗方法,避免误诊。方法分析1例膀胱嗜铬细胞瘤的临床资料,并复习相关文献。结果患者因无痛性肉眼血尿就诊,血压正常,术前膀胱镜肿物活检病理报告为膀胱癌,拟行膀胱全切术。术中触及膀胱肿瘤时血压明显波动,复查术前活检标本确诊嗜铬细胞瘤,行膀胱部分切除术,术后病理亦证实诊断,随访2年恢复良好。结论 B超、CT等对膀胱嗜铬细胞瘤诊断价值有限,病史、膀胱镜检查可疑本病,术中触及肿物时出现血压明显波动的现象强烈提示本病可能。

Objective To explore the diagnosis and treatment of pheochromocytoma of bladder in order to avoid misdiagnosis.Methods Clinical data of 1 patient with pheochromocytoma of bladder was analyzed,and relevant literature was also reviewed.Results The patient was admitted for painless gross hematuria with normal blood pressure,and bladder total resection was to be performed because of bladder cancer by preoperative neoplasm biopsy under cystoscope.The obvious blood pressure fluctuation was recorded when a bladder tumor was touched during the surgery,and then the preoperative biopsy specimens were double-checked,and the pheochromocytoma of bladder was confirmed.Bladder ablation was performed,and postoperative pathology examination was confirmed the diagnosis.The patient recovered well as a two-year follow-up showed.Conclusion Values of Ultrasound and CT in diagnosis of pheochromocytoma of bladder are limited.The disease should be suspected when results of case history and cystoscopy give a strong hints,and blood pressure fluctuation is found during the surgery.
