舌癌根治肌皮瓣修复后患者突然窒息的研究

Clinical studies on sudden asphyxia after combined management of radical neck dissection, glossectomy, mandibulectomy and repairing myocutaneous flaps

目的 研究舌颌颈联合根治肌皮瓣修复后的患者突然窒息的主要原因。方法 作者从 1989～ 1992年对 36例舌颌颈联合根治肌皮瓣修复术后的患者进行研究 ,其中 4例发生窒息并且死亡。1.对这类病人术后进行全面监测 ;2 .对这类患者咽腔进行测量 ,并与正常人进行对照。结果 造成这类病人术后突然窒息的主要原因是咽腔过于狭窄和低血钾。结论 我科制定了相应的预防措施并应用于临床实践。在 1993～ 1997年所进行的这类手术共5 1例 。

WT5”BZ]Objective The author have studied 36 patients from 1989 to 1992 year who underwent combined management of glossectomy, mandibulectomy, radical neck dissection and repairing myocutaneous flap in order to find out the causes of sudden asphyxia, Of the 36 cases, 4 patients suffered asphyxia and four were dead. Methods Procedures used in this study included:(1)comprehensive monitoring of the patients after surgery and (2) measuring the size of laryngopharynx and comparing the data with that form the normal subjects. Results The results showd that sudden asphyxia was caused by extremely strictured laryngopharynx and hypokalemia. Conclusion Based on this findings, appropriate preventive measures have been taken and put into clinical practice in our department. In the period of 1993 to 1997, such operations were performed on 51 patients, none of them suffered sudden asphyxia.
