慢性阻塞性肺疾病合并呼吸衰竭临床治疗效果分析

The Analysis of Treatment Effect in Chronic Obstructive Pulmonary Disease Combined with Respiratory Insufficiency

目的探究慢性阻塞性肺疾病(COPD)合并呼吸衰竭临床治疗效果分析。方法选择我院2011年2月—2012年2月收治的120例COPD合并呼吸衰竭患者,随机分为对照组和观察组,各60例。对照组给予常规治疗,观察组在此基础上联合无创正压机械通气治疗,治疗1d,观察两组患者各临床检验结果、临床疗效及不良反应。结果两组患者治疗1d后氧分压(PaO2)、二氧化碳分压(PaCO2)、pH、心率(HR)、呼吸(R)等临床检验结果均较治疗前好转,但观察组的治疗效果优于对照组,差异有统计学意义(P<0.05)。观察组的总有效率(91.7%)高于对照组(76.7%),差异有统计学意义(P<0.05)。两组均未发生不良反应。结论对慢性阻塞性肺疾病合并呼吸衰竭患者应用无创通气法治疗效果显著,且不良反应少,值得临床应用。

Objective To explore the clinical efficacy of patients with chronic obstructive pulmonary disease and re- spiratory insufficiency. Methods The 120 cases of patients with respiratory failure in our hospital from February 2011 to Febru- ary 2012 were elected to the group. The patients were randomly divided into control group and observation group, 60 cases in each. The control group received conventional treatment, the observation group combined noninvasive positive pressure ventilation treatment on this basis. After treatment of one day, two group patients were observed the various clinical test results, clinical ef- ficacy and adverse reactions. ResultsThe clinical examination PaO2, PaCO2, PH, HR, R of two group patients after treatment I d were better than before treatment, but the treatment effect of the observation group were better than the control group （P 〈 0. 05） . The total efficiency of the observation group （91.7%） was higher than the control group （76. 7%, P 〈 0. 05 ） . Two groups did not have adverse reactions as well. Conclusion The effects of chronic obstructive pulmonary disease and respiratory failure treatment of non - invasive ventilation in patients were remarkable and adverse reaction was less. So it is worthy to clinical application.
