摘要
目的 探讨胰岛素样生长因子Ⅰ (IGF Ⅰ )及胰岛素样生长因子结合蛋白 1(IGFBP 1)与超排卵周期卵泡发育的关系。方法 采用酶联免疫吸附试验 (ELISA)和放射免疫法 (RIA) ,检测 32例体外受精 胚胎移植 (IVF ET)患者超排卵前血清、取卵时平均直径 18mm以上卵泡的卵泡液及同期血清中IGF Ⅰ、IGFBP 1、雌二醇 (E2 )的水平。结果 血清IGFBP 1水平取卵时为 (3 5± 1 0 ) μg/L ,显著高于超排卵前的 (2 1± 0 5 ) μg/L (P <0 0 5 ) ,卵泡液内的平均水平为 (77 7± 2 6 7) μg/L ,显著高于同期血清中的水平 [(3 5± 1 0 ) μg/L](P <0 0 0 1)。取卵时血清IGFBP 1水平与卵泡发育数 [(9 4± 6 3)个 ]呈正相关 (r =0 48,P <0 0 5 )。但超排卵前、后血清IGF Ⅰ水平 [分别为 (10 3 7± 39 2 ) μg/L、(10 5 9± 36 8) μg/L]改变及其在取卵时卵泡液内的平均水平 [(10 8 9± 19 1) μg/L],与同期血清中的水平比较 ,差异均无显著性 (P >0 0 5 )。取卵时血清IGFBP 1水平与E2 水平 [(3 2 93± 136 1)pmol/L]呈正相关 (r=0 41,P <0 0 5 )。结论 超排卵前、后体内总IGF Ⅰ水平不变 ,但IGFBP 1改变可能调节体内游离IGF Ⅰ的水平 ,参与对卵泡发育的调控。
Objective To investigate the effects of insulin-like growth factor Ⅰ (IGF-Ⅰ) and insulin-like growth factor binding-protein 1 (IGFBP-1) on development of ovarian follicles during gonadotropin stimulation in in-vitro fertilization (IVF) program. Methods Enzyme-linked immunosorbent assay (ELISA) and radioimmunoassay were used to determine the levels of IGF-Ⅰ, IGFBP-1 and estradiol (E 2) in serum and follicular fluid obtained during oocyte pick-up (OPU) in 32 IVF-embryo transfer cycles. Results (1) Serum IGFBP-1 levels increased with growth of follicles [(3 5±1 0) μg/L during OPU, (2 1±0 5) μg/L before stimulation, P <0 05]; follicular fluid (FF) IGFBP-1 levels were significantly higher than those of matched serum levels [(77 7± 26 7) μg/L, (3 5±1 0) μg/L respectively P <0 001]; Serum IGFBP-1 levels were positively correlated with developing of follicle numbers ( r =0 48, P <0 05). (2) There were no significant changes of serum IGF-Ⅰ levels before and after superovulation, neither was there between serum and follicle fluid [(103 7±39 2) μg/L, (105 9±36 8) μg/L,(108 9±19 1) μg/L respectively P >0 05]. Significant positive correlation was found between serum IGFBP-1 and serum E 2 [(3 5±1 0) μg/L, (3 293± 1 361) pmol/L, r =0 41, P <0 05). Conclusions During superovulation serum total IGF-Ⅰ levels do not change. However changes of serum IGFBP-1 levels may play a complementary role in regulation of follicular development by altering free IGF-Ⅰ levels.
