Perpendicular Double Plate Fixation to Treat AO Type C Fractures of the Distal Humerus

摘要 目的:探讨经尺骨鹰嘴截骨垂直双钢板固定治疗C型肱骨远端骨折的方法和临床疗效。方法:2005年7月--2012年11月采用经尺骨鹰嘴截骨入路垂直双钢板固定治疗c型肱骨远端骨折36例。骨折按照A0/ASIF分类:G8例,C216例,C312例。结果:36例获得随访,时间5~28个月,平均14.3个月。骨折全部愈合,愈合时间12~20周,平均15.8周,无切口感染、内固定松动及断裂等。疗效参照改良的Cassebaum法评估:优10例,良19例,可3例,差4例;优良率80.6%。结论:经尺骨鹰嘴截骨垂直双钢板固定治疗c型肱骨远端骨折,稳定性好,可早期进行关节功能锻炼,疗效好。 Objective:To explore the surgical managements and clinical effects in patients with AO type C fractures of the distal humerus treated using perpendicular double plating methods through olecranon osteotomy approach. Meth- ods: From July 2005 to November 2012,36 patients with type C fractures of the distal humerus were treated using per- pendicular double plating method through olecranon osteotomy approach. According to AO/ASIF classification, there were 8 cases of type Ca, 16 cases of type C4,12 cases of type Ca. Results: All patients were followed up for 5~28 months (average 14. 3 months). All fractures healed. The healing time was 12~20 weeks(average 15.8 weeks). There was no incision infection and failure of osteosynthesis. According to Cassebaum scoring system, there were 10 excellent cases, 19 good,3 fair and 4 poor. The excellent and good rate was 80. 6%. Conclusion.-Perpendicular double plating through olecranon osteotomy approach has advantages of stable fixation, early functional exercise and good clini- cal effects, is an ideal methods for treating AO type C fractures of the distal humerus.
