Surgical treatment of congenital heart diseases in 160 infants

目的 总结连续 16 0例婴儿先天性心脏病的外科治疗经验。方法 病婴系 2～ 12月龄 ,平均 (7 3± 2 9)月龄的男婴 96例 ,女婴 6 4例。体重 3～ 11kg ,平均 (6 7± 1 4)kg。心胸比率 0 45～ 1 0 0 ,平均 0 6 1± 0 10。 6 3例有不同程度肺动脉高压。 10 3例有 2种以上病变。 15例常温下行未闭动脉导管缝扎术 ,其余 145例均在全身麻醉、低温、体外循环下行心内畸形一期根治术。结果 术后 30d内死亡 13例 (8 1% ) ;发生各种并发症 2 7例 (16 9% )。无远期死亡病例 ,发生远期并发症 5例 (3 4% )。结论 婴儿先天性心脏病早发现、早诊断、早治疗 。

Objective To review the experience of the surgical treatment of congenital heart diseases in 160 infants. Method From June 1993 to December 1998,160 infants were operated on for various congenital heart diseases. There were 96 males and 64 females. Their ages ranged from 2 to 12 (mean ages 7 3±2 9) months, the body weights were (6 7±1 4)?kg, (ranged 3 to 11)?kg. The cardiac and thoracic ratio before operation was 0 45～1 00(0 61±0 10). 63 infants had various degrees of pulmonary hypertension and 103 infants were associated with two or more cardiac anomalies. 15 infants had ligation of patent ductus arteriosus under normothermic anaesthesia and 145 infants were operated on for various cardiac anomalies under hypothermic cardiopulmonary bypass. Result The operative mortality within 30 days was 8 1% (13 cases) and the morbidity was 16 9%(27 cases). During follow up, no deaths occurred and complications occurred in 5 cases. Conclusion The congenital heart diseases in infants should be diagnosed as early as possible. The early surgical intervention is satisfactory.
