地佐辛对鞘内舒芬太尼分娩镇痛所致瘙痒的治疗作用 被引量:7

摘要 目的探讨地佐辛对鞘内舒芬太尼分娩镇痛所致瘙痒的治疗作用。方法选取90例美国麻醉师协会分级I-Ⅱ级、单胎、足月(≥37周)、枕前位妊娠拟经阴道分娩,行鞘内注射舒芬太尼5ug分娩镇痛后24h内瘙痒的产妇,完全随机分成2组(每组45例)。地佐辛组静脉注射地佐辛5mg(1m1),0.9%氯化钠注射液组静脉注射0.9%氯化钠注射液1ml。瘙痒评分、镇痛评分、镇静评分每5分钟记录1次,记录30min。结果地佐辛组瘙痒的治疗有效率明显高于0.9%氯化钠注射液组[80.0%(36/45)比15.6%(7/45),P〈0.01],2组的镇痛评分比较差异无统计学意义[(2.2±0.5)分比(2.0±0.3)分](P〉0.05),2组的镇静评分比较差异有统计学意义(P〈0.05)。结论地佐辛5mg静脉注射可以有效地治疗鞘内舒芬太尼分娩镇痛所引起的瘙痒。 Objective To investigate the effect of dezocine on the spinal sulfentanyl-induced pruritus during labor analgesia. Methods Ninety patients with american society of anesthesiologists physical status 1-2, nulliparous and undergoing a spontaneous labor ( ≥37 weeks of gestation ) with a live, singleton fetus, receiving spinal sulfentanyl-induced pruritus during labor analgesia ( in postoperative 24 h ) were randomly allocated to receive 5 mg dezocine (1 ml) (dezocine group, 45 cases)or 1 ml 0.9% sodium chloride injection (0.9% sodium chloride injection group, 45 cases)intravenously. Pruritus, sedative and the level of postoperative pain were assessed every 5 min till the end of the study period (30 min). Results The treatment success rate in the dezocine group was higher than that in the 0.9% sodium chloride injection group [ 80.0% (36/45) vs 15.6% (7/45), P 〈 0.01 ]. Other side effects, such as decreased analgesia, were not significantly different between groups. Sedation scores between the two groups was statistically significant [ ( 2. 2 ± 0. 5 ) scores vs ( 2. 0 ± 0. 3 ) scores ]. Conclusion Dezocine is an efficient drug treatment for spinal sulfentanyl-induced pruritus during labor analgesia.
