摘要
目的:通过检测高迁移率族蛋白B1(high mobility group box1,HMGB1)在甲状腺乳头状癌组织中的表达情况,并分析与临床病理特征之间的相关性,探讨其在甲状腺乳头状癌中发生、发展及转移等的意义。方法:选取2010-01-18-2012-11-23潍坊医学院附属医院50例临床确诊的甲状腺乳头状癌组织及20例结节性甲状腺肿组织病理蜡块标本,采用免疫组织化学和免疫荧光检测HMGB1的表达,并分析其与甲状腺乳头状癌临床病理特征的关系。结果:HMGB1蛋白主要表达于细胞质和细胞核中。甲状腺癌组织和结节性甲状腺肿组织中HMGB1阳性表达率分别为74.0%(37/50)和15.0%(3/20),P<0.05。甲状腺乳头状癌组织中HMGB1蛋白阳性表达与年龄(χ2=0.049,P=0.825)、性别(χ2=0.002,P=0.486)、肿瘤大小(χ2=0.485,P=0.968)及TNM分期(χ2=1.006,P=0.316)均无关;与包膜浸润(χ2=4.299,P=0.038)和淋巴结转移(χ2=5.889,P=0.015)密切相关。多因素Logistic回归分析显示,包膜浸润危险度(OR)最大。结论:HMGB1在甲状腺乳头状癌中高表达,预示着HMGB1可能参与肿瘤的发生、发展及转移。
OBJECTIVE:To detect the expression of HMGB1 protein in papillary thyroid carcinoma and analyze the correlation of its clinicopathologic characteristics, and investigate its significance in the process of occurrence, develop ment, metastasis of thyroid papillary carcinoma. METHODS:2010-01-18--2012-11 23 Affiliated Hospital of Weifang Medi- cal College, from 50 patients with thyroid papillary carcinoma and 20 patients with nodular goiter, via immunohistochemis- try and immunofluorescence to detecte the expression of HMGB1 and analyze the clinicopathologic characteristics of thy roid papillary carcinoma. RESULTS.. HMGB1 protein was mainly expressed in the cytoplasm and nucleus. Positive expres sion of HMGB1 was 74.0%(37/50) in carcinoma tissue,and that was 15.0%(3/20) in the nodular goiter. HMGB1 pro rein in papillary thyroid carcinoma tissue was significantly higher than that in nodular goiter (P〈0.05) ,and HMGB1 pro tein expression were unrelated with age(x2 =0. 049,P=0. 825),gender(x2 --0. 002,P=0. 486) ,tumor size (X2 =0. 485, P=0. 968) ,TNM stage(x2 =1. 006,P 0. 316). Multivariate logistic regression analysis demonstrated positive expression of HMGB1 was closely related with capsular infiltration(x2 = 4. 299, P= 0. 038) and lymph node metastasis (X2 = 5. 889, P=0. 015). CONCLUSION: High expression of HMGB1 indicates that HMGB1 may be involved in the process of occur rence, development, and metastasis in papillary thyroid carcinoma.
