摘要
目的探讨发音障碍严重指数(DSI)在声带息肉患者中的应用价值。方法通过DIVAS2.5声学分析软件对25例声带息肉患者(病例组)术前及术后1周、2周、3周进行DSI检测及嗓音声学分析,与30例正常者(对照组)进行比较,分析DSI的特征及可靠性。结果声带息肉患者术前均有较高的基频微扰、振幅微扰及较低的DSI,结果均能提示嗓音功能受损。声带息肉患者术后1周与术前比较,基频微扰、振幅微扰无明显改变,差异无统计学意义;DSI升高,差异有统计学意义(P<0.05),提示术后1周嗓音功能得到改善,而且DSI能敏感反映嗓音功能的变化。术后2周基频微扰、振幅微扰与对照组差异无统计学意义(P>0.05),而DSI差异仍有统计学意义(P<0.05),提示嗓音功能未完全正常,DSI能敏感反映嗓音状态。术后3周,各指标与对照组差异均无统计学意义(P>0.05),提示嗓音完全恢复。结论在25例声带息肉患者中,DSI较扰动分析能够更敏感地反映嗓音变化情况。
The detection of dysphonia severity index and its significance in 25 patients with polyp of vocal cord
Objective To explore the objective detection of Dysphonia Severity Index(DSI) and its significance in patients with polyp of vocal cord. Methods The DSI had been completed in 25 patients with polyp of vocal cord before the operation and one,two and three weeks after the surgery respectively. Thirty normal subjects were enrolled as controls. In the meantime, the voice acoustic analysis had been completed in these patients by acoustic analysis of DIVAS2.5. Both the feature of DSI and its reliability had been evaluated simultaneously. Results Jitter and Shimmer were supernal, but with low-grade DSI, showing the function of voice in these patients were dysphonia. There was no significant difference between preoperation and one week postoperation in the Jitter and Shimmer( P〉0.05). In contrast the DSI increased (P〈0.05).These showed that the function of voice in these patients had been meliorate and the DSI was a sensitive index. After two weeks, there was no significant difference between control group and experimental group in the Jitter and Shimmer(P〉0.05),but there was between control group and experimental group in DSI (P〈0.05). These showed that the function of voice in these patients had not been completely meliorated, the DSI could sensitively reflect the function of voice .After three weeks, there were no significant difference in the above indexes between control group and experimental group,it showed that the function of voice had been normal.
