肝脏转氨酶升高在儿童腹部闭合性损伤评估中的作用 被引量:7

摘要 目的:探讨儿童腹部闭合性损伤后血清谷草转氨酶(AST)和谷丙转氨酶(ALT)升高在损伤程度和范围预测中的作用。方法:采用回顾性研究,收集2008年1月—2012年12月因怀疑腹部闭合性损伤就诊的87例儿童患者病历资料,其中49例患者确诊为腹部闭合性损伤,按患者是否存在腹部闭合性损伤(I组和II组)以及是否经影像学检查确诊腹腔内损伤(Ia组和Ib组)分组,比较患者血清AST和ALT水平。结果:I组和II组血清AST和ALT水平具有统计学差异[(145.3±180.4)U/L vs.(34.8±10)U/L和(84±119.8)U/L vs.(25.1±13.8)U/L,P<0.05)],Ia组血清AST和ALT水平分别为(333.6±283.8)U/L和(197.5±192.5)U/L,高于Ib组的(84.2±55.9)U/L和(43±29.8)U/L(P<0.001);影像学确诊的腹腔内损伤患者血清AST和ALT水平分别>110.5 U/L和63.5 U/L,肝损伤患者血清AST和ALT水平分别>500 U/L和300 U/L;影像学确诊的腹腔内损伤与血清AST(>110.5 U/L)和ALT(>63.5 U/L)水平呈显著正相关(r=0.63和0.58,P<0.05)。结论:腹部闭合性损伤患者血清AST和ALT水平显著升高,AST>110.5 U/L和ALT>63.5 U/L提示存在腹腔内损伤,如果AST和ALT水平进一步升高则应考虑严重肝损伤。
