摘要 目的探讨Meckel腔(MC)肿瘤的诊断及手术治疗效果。方法对我院收治的27例MC肿瘤病人的临床资料进行回顾性分析;其中局限在MC内17例,长满MC稍突向后颅窝6例,长满MC并发展到岩斜区2例,长满MC并经圆孔及卵圆孔突向颅外2例;均采用改良的颞下-硬膜内入路显微手术治疗。结果肿瘤全切除26例,次全切除1例。病理学检查结果为三叉神经鞘瘤18例,脑膜瘤6例,表皮样囊肿3例。所有患者术后随访3个月至4年,症状缓解或消失,复查MRI未见肿瘤复发。结论磁共振平扫加增强是MC肿瘤诊断首选检查方法,改良的颞下-硬膜内入路显微手术是治疗MC肿瘤安全可靠的方法。 Objective To explore the diagnosis and microsurgical treatment of Meckel's cave (MC) tumors. Method The clinical data of 27 consecutive patients with MC tumor who underwent microsurgery via infratemporosubdural approach were analyzed retrospectively. Results The tumors were located within the MC in 20 patients, invaded the posterior cranial fossa in 5, invaded the foramen rotundum and foramen ovale in 2. The total resection of the tumors was achieved in 26 patients and subtotal in 1. According to the postoperative pathological findings, 18 tumors were trigeminal neurinoma, 6 meningioma and 3 epidermoid cyst. The follow-up time ranged from 3 months to 4 years. All the symptoms resolved in all the patients. No tumors recurred in all patients. Conclusion The microsurgery via infratemporosubdural approach is an effective treatment of MC tumors.
