摘要
目的探讨超声瞬时波强技术(wave intensity,WI)在评价系统性红斑狼疮患者左心室功能中的临床应用价值。方法对42例(病例组)系统性红斑狼疮活动期患者和30例(对照组)健康受检者进行颈部WI检查,获得WI曲线,测量W1、W2值;应用常规超声心动图检查,测量左室射血分数(LVEF)、左室短轴缩短率(LVFS)及二尖瓣口血流峰值速度E峰、A峰。结果①病例组和对照组之间E/A值、LVEF及LVFS无明显差异(P>0.05);病例组和对照组W1值差异不大,无统计学意义(P>0.05)。②病例组W2值低于正常对照组,且差异有统计学意义(P<0.05)。③病例组抗心磷脂抗体(anticardiolipin antibody,ACA)阳性者W2值明显低于ACA阴性者(P<0.01)。结论 WI技术结合ACA可以敏感检测到SLE患者左心功能的早期损伤。
Objective To evaluate the clinical value of wave intensity (WI) combined anticardiolipin antibody (ACA) in assessing the left ventricular function in patients with systemic lupus erythematosus (SLE). Methods Forty-two SLE patients and forty control subjects were enrolled. Left ventricular ejection fraction (LVEF), left ventricular fraction of shortening (LVFS), peak early diastolic velocity(E) and late diastolic velocity(A) were measured by using conventional echocardiography. WI curve of carotid artery was obtained by using wave intensity (WI) and W1 and WE values were measured. Results ①There was no difference in E/A, LVEF and LVFS among SLE patients and controls (P 〉 0. 05 ). The W1 peak value had no significant difference. ②The W2 value of SLE patients were lower than those of controls and there were significant differences ( P 〈 0.05 ). ③Tbe W2 value of ACA positive patients were significantly lower than those of ACA negative ones ( P 〈 0. 01 ). Conclusion WI combined with ACA can detect myocardial dysfunction of SLE sensitively at the early stage.
