摘要
文章梳理了荷兰、英国和美国三个国家30年以来的医改历程和经验,发现这些国家的医改理念趋于相似。荷兰从政府管制的模式走向以管理型竞争为目标。英国从国家主导模式走向引入内部市场制,并正在逐步扩大选择与竞争。美国则在市场主导的框架下,开始强化政府的作用。这些国家医改体现了共同的方向:在筹资和医疗卫生体制的宏观管理上,发挥政府的组织领导功能,而在医疗卫生的实际生产和微观管理过程中,政府放手和退出,并且都在不同程度上实践了有管理的竞争理论。我国应当借鉴这些国家的经验,并以有管理的竞争为目标,开展我国的卫生事业改革与建设。
The experiences of healthcare reform in Netherlands, Britain and America in the last 30 years were studied. From which, it can be concluded that the ideas of healthcare reform tend to be similar in these countries. The governmental regulation .mode has been transformed into regulated competition in Netherlands. The inner market system has been established in Britain, which is encouraging more choices and more competition. The function of government has been reinforced within the market framework. Of all these countries, the governmental organization and leadership function has been strengthened in financing and macro management system. And the government has quitted from the practices of healthcare and micro management of medical health, which practiced the managed competition theory. These experiences should be referred in China to improve the healthcare reform.
