摘要
目的 支气管肺泡灌洗术 (BAL)能否治疗矽肺仍有异议 ,进行其实验研究 ,可为临床治疗提供科学依据。方法 观察各组大鼠BAL后的组织学改变 ,用免疫细胞化学法观察其肺巨噬细胞数量的变化。结果 染尘后的大鼠肺 ,经灌洗后肺泡腔内容物减少 ,肺泡隔厚度变薄 ,肺组织的细胞密度较前下降 (包括肺泡腔及肺泡隔中的巨噬细胞数 ) ,且差别有显著意义 (P <0 .0 1)。肺间质的粉尘 ,可通过清除肺泡隔内的巨噬细胞而清出。结论 BAL对延缓矽肺病变的发生、发展起积极作用。
Objective There are different opinions on BAL therapy. The theraptic effect of BAL on silicotic rats was studied in this experiment.Methods The general histologic changes were observed, and the number of pulmonary alveolar macrophage (PAM) stained with immunocytochemistry was calculated.Results The amount of PAM either within the alveolar septa or alveolar cavity decreased significantly in BAL treated rats comparing with the untreated rats, as well as their fluid exudation in alveoli.Conclusion Some of the dust phagocytes in pulmonary mesenchyme could be cleared out from alveolar septum to cavity by BAL, which might play an active role in alleviating the formation of fibrous silicotic nodule.
