无晶状体难治性青光眼的联合手术治疗

Combined surgery to obstinate glaucoma of aphacia.

目的 探讨无晶状体难治性青光眼小梁切除联合玻璃体抽吸及丝裂霉素应用的疗效。方法 14例术中应用 0 4mg/ml丝裂霉素结膜瓣下及巩膜瓣下湿敷。术后随访 1年。结果 14例中 10例术后视力提高 ,8例恢复有用视力 ,3例术后视力保护不变 ,1例术后视力下降。 10例眼压控制在 2 0 5 5mmHg以下 ,3例术后需应用药物治疗 ,1例眼压 >31 5 2mmHg。结论 无晶状体难治性青光眼行小梁切除联合玻璃体抽吸及应用丝裂霉素治疗 ,能提高手术成功率。

To investigate the efficacy of the operation of trabeculectomy combined with vitreus aspiration and the use of mitomycin C to obstinate glaucoma of aphacia.Methods Combined trabeculectomy with vitreum aseration and the use of mitomycin C during the operation for the treatment of obstinate glaucoma of aphacia for 14 cases.Results Visual acuity was improved in 10 cases, restored in 8 cases, unchanged in 3 cases, worse in 1 case. The ocular tension was controlled less than 20.55 mmHg in 10 cases. 3 cases still need the use of drug after the operation. The intraocular tension was >31 52 mmHg on 1 case. Conclusion That combined trabeculectomy with vitreus aspiration and the use of mitomycin C for the treatment of obstinate glaucoma of aphacia can improve the success rates of the operation and retain the visual acuity for the patients.
