静脉药物配置中心差错分析与防控措施

Errors control and analysis in pharmacy intravenous admixture services

目的减少静脉药物配置中心的差错发生率,保障患者用药安全。方法通过分析本院静脉药物配置中心的主要工作环节出现的差错,并采取相应的防控措施。结果采取措施后静脉药物配置中心差错发生率由2010年0.076%降至2012年0.030%,其中未出门差错率由0.074 5%降至0.029 5%,而出门差错率由0.001 5%降至0.000 5%。结论通过对药师审方、摆药、药品配置、成品核对等环节采取防控措施,能有效的降低静脉药物配置中心的差错率,确保了临床用药的安全。

Objective To reduce the errors in Pharmacy Intravenous Admixture Services （PIVAS） to ensure the safety of patients with medications. Methods The main aspects of work in PIVAS in our hospital were analyzed, and the correspond- ing prevention and control measures taken. Results The error rate in PIVAS of our hospital decreased from 0.076% in 2010 to 0.030% in 2012 after prevention and control measures. The error rate before leaving the PIVAS decreased from 0.074 5% to 0.029 5%, while the error rate after leaving the PIVAS decreased from 0.0015% to 0.0005%.Conclusion Prevention and control measures include double checking prescriptions, pendulum drugs, configuration drugs and finished products of PIVAS, which can effectively reduce the rate of errors and ensure the safety of clinical drug use.
