后房人工晶状体植入术500例术中术后并发症

Clinical investigation of intraoperative and postoperative complications in 500 cases of posterior chamber IOL implantation in cataract.

目的 探讨白内障囊外摘出人工晶状体植入术的术中术后并发症发生的原因及预防处理方法。方法 回顾了 1993～ 1997年常规切口白内障囊外摘出后房型人工晶状体植入术5 0 0例 ,并加以分析。结果 术中后囊破裂 11眼 ,玻璃体溢出 9眼 ,虹膜根部断离 11眼 ,前房积血 14眼 ;术后发生不同程度的角膜内皮水肿者 171眼 ,后囊浑浊 30眼 ,眼压增高 15眼。术后 1～2周裸眼视力 0 5以上者 170眼 ,低于 0 5者 330眼。结论 提高操作技巧 ,减少前房内操作 ,可有效地减少并发症的发生 。

To research the causes and prophylatic methods of the intra and post operative complications of extracapsular cataract extraction (ECCE) and intraocular lens implantation.Methods 500 cases from 1993 to 1997 of ECCE and intraocular lens implantation with routine incision were reviewed and analysed.Results Of the 500 cases,posterior capsule repture in 11 eyes,proplapse of vitreous in 9 eyes,hyphema in 14 eyes during the operation.And corneal opacity of various degree in 171 eyes,posterior capsule opacity in 30 eyes,increased tension in 15 eyes post operatively.171 eyes had the visual acuities of 0.5 or better,and 33 eyes were lower than 0.5,1～2 weeks after the surgery.Conclusion Improving the surgerical skill and reducing the operation in anterior chamber may reduce the complication and improve the postoperative visual acuity effectively.
