跟骨Gissane’s角的测量及其临床意义

The measurement of Calcaneal Gissane's angle and its clinical significance

目的 :研究跟骨Gissane’s角的数值范围、跟骨骨折后Gissane’s角的改变以及Gissane’s角与跟骨骨折预后间的关系。方法 :测量 12 0例正常跟骨侧位x线平片和 30例跟骨骨折侧位x平片的Gissane’s角 ;随访 35例CrabyⅡ型以上跟骨骨折患者进行临床Maryland评分。结果 :正常跟骨的Gissane’s角为12 3 .8°± 8.7°(10 3 .3°～ 14 6 .8°) ,无性别差异 ;跟骨骨折后Gissane’s角为 131.2°± 10 .2°(12 4.8°～ 15 7.5°) ,明显大于正常Gissane’s角 ;Gissane’s角正常的跟骨骨折治疗疗效Maryland评分优良率为 6 8.5 % ,而Gissane’s角不正常的跟骨骨折Maryland评分优良率仅为 5 .8%。结论 :Gissane’s角的正常范围为 110°～ 14 0° ,骨折后呈增大趋势 ;Gissane’s角不是决定跟骨骨折预后的主要因素 ,但跟骨骨折后应尽可能恢复Gissane’s角的正常。

Objective: To study the normal range of calcaneal Gissane's angle, its changes after fracture and the relationship between the Gissane's angle and the prognosis of calcaneal fracture. Methods: The Gissanes's angles were measured on the lateral X ray of 120 normal and 30 fracture calcaneus.The clinical results of 35 calcaneal fracture patients were evaluated with Maryland's standard.Results: Normal Gissane's angle is 123.8°±8.7°, and it increase to 131.2°±10.2° after fracture.Evaluated with Maryland's standard, 68.5% fracture patients with normal Gissane's angle have a good results, while it is 5.8% for the fracture patients with abnormal Gissane's angle.Conclusion: The normal range of Gissane's angle is 110°～140°.Though Gissane's angle isn't the major factor to affect the cure of calcaneal fracture, it's better to restore the Gissane's angle into normal range after calcaneal fracture.
