时差成像监测在评估胚胎发育潜能中的作用 被引量:2

摘要 选择性单胚胎移植已逐步在临床应用中能够接受,提高胚胎着床率的需求因而变得至关重要,选择最具发育潜能的胚胎仍然是辅助生殖技术的一个重大挑战。在给定时间点胚胎形态和发育阶段与胚胎发育潜能是紧密相关的,因而,形态参数的评价仍然是评价胚胎发育潜力的首选方法。最近,胚胎发育的动态评估作为评估胚胎潜能的一种工具受到更多的关注。时差成像设备的引入及在人类胚胎的批准使用为胚胎连续监测提供了新的临床机会,实现了对已知形态学参数的灵活评价,并可能引入新的胚胎发育能力动态指标。由于缺乏较大的随机临床研究,因此,还有待阐明胚胎选择是否使用动态参数改善临床结果以及哪些参数具有意义。本文总结了当前关于胚胎发育能力动态标记的进展并讨论了时差成像监测分析在胚胎评估和选择中潜在的临床作用。 As elective transfer of a single embryo (eSET) becomes increasingly accepted, selecting the most competent embryo there fore becomes crucial and constitutes a major challenge in assisted reproductive technology. Embryo morphology and developmental stage at given time points are closely correlated with developmental competence and assessment of morphological parameters thus re- mains the preferred way of evaluating embryonic potential. Recently, more attention has been given to the assessment of dynamic em- bryo development as a tool for evaluating embryonic potential. The introduction of time - lapse equipment approved for use on human embryos offers novel clinical opportunities for continuous monitoring of embryos. Due to lack of larger, randomized clinical studies it re- mains to be elucidated whether embryo selection using dynamic parameters improves clinical outcome and which parameters are of sig- nificance. This review summarizes the current knowledge about dynamic markers of viability and discusses the potential clinical role of time -lapse analysis in embryo assessment and selection.
