摘要
目的 探讨终池持续引流对急性颅脑患者脑脊液 (CSF)成份的影响。方法 将 80例急性颅脑损伤患者分为两组 ,终池持续引流组 40例 ,应用Seldinger穿刺技术穿刺终池 ,留置深静脉留置管于终池持续引流CSF ;对照组 40例 ,应用常规腰穿放CSF的方法。结果 终池持续引流组第 1天 ,前 3、5天的CSF引流量均明显多于对照组 (P <0 .0 1)。第 1天CSF中红细胞 (RBC)及蛋白质 (TP)的含量和对照组无显著性差异 (P >0 .0 5 )。第 3天及第 5天CSF中RBC及TP的含量明显低于对照组 (P <0 .0 1)。终池持续引流组无 1例发生交通性脑积水 ,而对照组有 2例因交通性脑积水行脑室腹腔分流术。结论 终池持续引流能够更快降低CSF中有害成分的含量 ,减轻继发性脑血管痉挛和脑损伤的程度 ,减少交通性脑积水的发生机会。
Objective To evaluate the influence of draining CSF in spinal subarachnoid on CSF component of patiients with acute head injury.Method 80 cases with acute head injury were divided into 2 groups,40 cases in group of continuous drainage using seldingers technqiue were put a central venous catheter into spinal subarachnoid for continuous draining CSF,40 cases in control group were used routine lumbar puncture.Results Continuous draining group drained out more CSF than the control group,the quantity of erythrocyte and protein in CSF in continuous draining group reduced more quickly than that in control group in the same time.Conclusions Continuous drainage of CSF can reduce secondary brain injure in patients with acute head injury.
