颈部开放性外伤的诊断与治疗

Diagnosis and Treatment of Open Injury of Neck

目的：探讨复杂颈部外伤的有效诊治方法。方法：对23例颈部外伤患者采用个体化综合治疗，其中12例行气管切开术。结果：23例患者中22例抢救成功。12例气管切开患者2周顺利拔管，3例喉挫裂伤患者3周后均予以拔管，本组病例随访6～12个月，无1例发生喉及气管狭窄，3例喉返神经损伤患者，1例1个月后声嘶好转，2例无改善。结论：对复杂颈部外伤要及时明确损伤的部位和程度，及时打通呼吸通道。

Objective：To discuss the effective methods of diagnosis and treatment on complex open injury of neck. Methods：23 pa- tients were treated by individualized comprehensive treatment, included 12 tracheotomy cases. Results ：22 cases were saved successfully. 12 tracheotomy cases were extubated after 2 weeks. 3 laryngeal contusion cases were extubated after 3 weeks. All the cases were followed for 6 - 12 months, no laryngostenosis and tracheostenosis happened. There were 3 cases of recurrent laryngeal nerve injury, one patientg hoarseness was improved and other 2 patients＇were not improved. Conclusions：It is very important to confirm the injured part and the degree for open injury of neck in time. Do the tracheotomy in time is also important.
