摘要
目的探讨三维超声容积对比成像C平面技术(Volume Contrast Imaging-C plane,VCI-C)与MRI两种影像学方法在评估胎儿小脑蚓部发育不良的价值.方法对小脑蚓部发育正常组胎儿30例和小脑蚓部发育不良组胎儿14例分别行VCI-C及MRI检查,测量胎儿小脑蚓部的高度、前后径和面积;测量脑干-小脑蚓部夹角(brainstem-vermis angle,BV角)、脑干-小脑幕夹角(brainstem-tentorium angle,BT角),采用配对t检验评估两种影像手段的一致性.结果 VCI-C技术与MRI两种方法在测量正常对照组胎儿小脑蚓部的高度、前后径和面积及BV角、BT角差异不具有统计学意义(P>0.05),可认为两种影像学方法在测量正常对照组胎儿小脑蚓部的高度、前后径和面积及BV角、BT角方面具有一致性;VCI-C技术与MRI两种方法测量小脑蚓部发育不良组胎儿小脑蚓部的高度、前后径和面积及BV角、BT角差异不具有统计学意义(P>0.05),可认为两种影像学方法在测量小脑蚓部发育不良组胎儿小脑蚓部的高度、前后径和面积及BV角、BT角方面具有一致性.结论两种方法在评估胎儿小脑蚓部发育不良方面具有一致性,VCI-C技术可作为产前诊断与鉴别诊断胎儿小脑蚓部发育不良的常规检查手段.
Objective To investigate the value of three-dimensional ultrasound volume contrast imaging-C plane (VCI-C) and MRI in assessing the development of fetuses with cerebellar vermis hypoplasia. Method VCI-C and MRI were performed in 30 fetuses with normal cerebeltar vermis and 14 fetuses with eerebellar vermis hypoplasia. The height, anteroposterior, area of fetal cerebellar vermis, BV angle and BT angle were measured respectively. Then, the consistency of VCI-C and MRI was evaluated by paired t test. Results No significant difference was found in height, anteroposterior and the area of vermis measured by two methods in control groups (P 〉 0.05). Thus, two methods showed good consistency for measuring height, anteroposterior and the area, BV angle and BT angle of fetal eerebellar vermis in control group. No significant difference was found in height,anteroposterior and the area of vermis measured by two methods in fetuses with cerebellar vermis hypoplasia( P 〉 0.05 ). Thus,two methods showed good consistency for measuring height, anteroposterior and the area, BV angle and BT angle of fetal cerebellar vermis in fetuses with cerebellar vermis hypoplasia. Conclusion VCI-C has good cnnsistency with MRI in assessing fetuses with eerebellar vermis hypoplasia. VCI-C technology can be routinely used for prenatal diagnosis and differential diagnosis of fetuses with cerehellar vermis hypoplasia.
