围手术期护理对直肠癌Dixon手术后并发直肠阴道瘘的预防效果分析

The analysis of preventive effect of perioperative nursing for rectovaginal fistula after Dixon opera- tion for rectal cancer

目的分析围手术期护理对直肠癌Dixon术后并发直肠阴道瘘的预防效果。方法选取2006年4月至2008年1月收治的行直肠癌Dixon术的患者94例,按照随机数字表分为观察组和对照组,每组47例,分别接受围手术期护理和常规护理,对比两组患者住院时间、痊愈时间、生存率、复发率及直肠阴道瘘发生率。结果观察组患者痊愈时间、直肠阴道瘘发生率均显著低于对照组(P<0.05);两组患者住院时间差异无统计学意义(P>0.05)。观察组患者复发率为10.6%(5/47),5年生存率为78.7%(37/47);对照组患者复发率为14.9%(7/47),5年生存率为51.1%(24/47),观察组患者生存率高于对照组,差异有统计学意义(P<0.05)。结论围手术期护理能够有效降低直肠癌Dixon术后并发直肠阴道瘘的风险,使患者的生活质量得以保证,应予以推广。

Objective To analyze preventive effec of perioperative nursing on rectal dixon postop- erative rectovaginal fistula. Methods 94 patients hospitalized from April 2006 to January 2008 receiving dixon rectal cancer surgery were randomly divided into the observation group and control group （47 cases respecitvely） according to a random number table. The control group admited to conventional care and the observation group perioperative care. Hospitalization time, recovery time, survival recurrence rate and incidence of reetovaginal fistula were compared between two groups. Results The recovery time, rectovaginal fistula was significantly lower than the control group （ P 〈 0. 05）. There were no significant difference in hospitalization time （ P 〉 0.05 ）. 5 cases in observation group of reeurrented （ recurrence rate 10. 6% ）, and 37 cases survived （ the 5 years survival rate was 78.7% ）. 7 cases in the control group recurrented （ the recurrence rate 14. 9% ） , and 24 cases survived （the 5 years survival rate was 51.1% ）. The survival rate of the observation gaoup was higher than the control group （P 〈 0. 05）. Conclusions Perioperative nursing can effectively reduce postoperative rectal Dixon rectovaginal fistula risk, guarantee patient＇s quality of life and survival, and should be promoted.
