黄芩苷联合干扰素α治疗慢性乙型肝炎疗效及对细胞免疫的影响

Baicalin combined with interferon α for treating patients with chronic hepatitis B and its effects on cellular immunity

目的研究黄芩苷联合干扰素α治疗慢性乙型肝炎临床疗效及对肝纤维化指标和细胞免疫的影响。方法 92例慢性乙型肝炎患者随机分为对照组(46例)和治疗组(50例),对照组给予干扰素α-2b治疗,治疗组用黄芩苷联合干扰素α-2b。治疗六个月后观察肝功能、肝纤维化指标、HBeAg及HBV-DNA阴转、CD+4、CD+8以及NK细胞亚群变化情况。结果两组患者肝功能、肝纤维化指标与治疗前相比均明显改善(P<0.01),但以治疗组改善更为显著(P<0.01);与对照组相比,治疗组患者HBeAg及HBV-DNA阴转率显著高于对照组(P<0.05,P<0.01);与对照组相比,治疗后治疗组CD+4T及NK细胞比例显著高于对照组,而CD+8T细胞比例显著低于对照组,CD+4/CD+8比值显著高于对照组(P<0.01)。结论黄芩苷和干扰素联用在抗炎、抗肝纤维化、促进HBeAg和HBV-DNA转阴及改善细胞免疫状态方面具有协同作用。

Objective To study the clinical efficacy of baicalin combined with interferon ot for treating patients with chronic hepatitis B （CHB） and its effects on indicators of hepatic fibrosis and cellular immunity. Methods 92 patients with CHB were randomly divided into the control group and the treatment group. The control group received the treatment of interferon alfa-2b（ n = 46）, while the treatment group received the combination therapy of interferon alfa-2b andbaicalin（n = 50）. Hepatic functions, indicators of hepatic fibrosis, status of cellular immunity, HBeAg and HBV-DNA were measured before and after treatment. Results Hepatic functions and indicators of hepatic fibrosis were improved significantly after the treatment for all the patients （ P 〈 0. 01, respectively）, but more significantly in the treatment group（P 〈0. 01, respectively）. The negative transformed rates of HBeAg and HBV-DNA in treatment group were significant higher than those in the control group （P 〈 0. 05, P 〈 0. 01, respectively）. Compared with the control group, the proportion of CD4＋ and NK increased significantly, proportion of CD8＋ decreased and the ratio of CD4/CD8＋ increased significantly after treatment in the treatment group （P 〈 0. 01 ）. Conclusion The combination of baicalin and interferon alfa-2b could improve the cellular immunity and the sera negative transformed rate, protect liver ceils from inflammation and prohibitive liver fibrosis.
