Frankel Ⅱ功能矫治器治疗下颌后缩合并开患者的颅颌面研究

Study on changes in cranio-facial by Frankel Ⅱ-appliances in the treatment of mandibular retrusion and opening malocclusion

目的:应用FrankelⅡ功能矫治器(FRⅡ)治疗生长发育高峰期下颌后缩合并开的患者,研究患者治疗前后颅颌面的变化情况。方法:对比研究20例下颌后缩合并开患者治疗前后的X线头影测量数据。结果:①治疗后患者上下颌切牙和磨牙均有伸长,上颌切牙突度减小,下颌切牙突度增加。②SNB、B-V治疗后数值增大,下颌骨的位置前移。下颌长度(Pg/V-Co/V)、下颌升支长(Go/X-Ar/X)治疗后均有增长,下颌骨在矢状和垂直方向都有所增加。③SN/MP、FMA、Y轴治疗前后无统计学意义,下颌平面没有发生顺时针旋转。④A-V、SNA治疗前后无统计学差异,上颌骨在矢状方向上变化很少。结论:①FrankelⅡ功能矫治器不仅改变了下颌骨的位置,同时还促进了下颌骨矢状方向和垂直方向的生长。下颌骨的位置、长度和上下颌牙齿轴倾度的共同变化使原来后缩面型得以改善,侧貌变得更加协调、美观。②开的解除主要是由于上下颌切牙的伸长和不良习惯的破除。③FrankelⅡ只适用于上颌骨正常或轻度前突的患者。

Objective To treat mandibular retrusion and Opening malocclusion at the peak stage of development by Frankel Ⅱ appliances and study the change in cranio-facial after orthodontic treatment Methods 20 patients with mandibular retrusion and Opening malocclusion were selected. Lateral cephalometric radiographs were measured and analysed for all the patients. Results ① Molars and incisor of maxillary and mandiblular elongated.The protrusion of upper incisor and lower incisor increased. ②The number of SNB,B-V decreased posttreatment,the position of mandible gone to previous.the length of mandible and ascending branch increased,the mandible of vertical and sagittal direction increased. ③There were no change the number of SN/MP,FMA,Y axis between before and after treatment. Mandibular plane did not rotate clockwise. ④There were no change the number of SN/MP,FMA,Y axis .The maxillaries did not change in sagittal direction. Conclusions ① Frankel Ⅱ appliances were not only change the position of mandibular but also promote the development in vertical and sagittal direction of mandible.Retrusive facial were improved by the change including the mandibular position,length and incisor protrusion of maxillary and mandiblular. the facial of patients become beautiful. ②Opening malocclusion were cured by the elongation of incisor and broke down of bad hadit. ③Frankel Ⅱ -appliances can be used in those with retrusive mandible and normal as well as mild protrusion maxilla.
