我院静脉滴注药物不合理处方分析

Analysis on unreasonable prescriptions of intravenous drip drugs in our hospital

目的促进临床合理用药,提高药物治疗的安全性和有效性。方法药师审查静脉滴注药物处方时,对不合理处方(医嘱)进行干预,并分类记录,分析。结果静脉滴注药物不合理处方主要存在溶媒选用不当、给药浓度不规范、多药配伍不当、给药方式不当、超剂量用药、重复给药等方面的问题。结论静脉药物配置中心为药师们提供了药学服务的平台,通过药师对医嘱的审核与干预,协助医师及时纠正临床静脉用药的问题处方,有效减少或避免临床不合理用药,提高临床用药的安全性与有效性。

Objective To promote rational drug use in clinical practice, improve the safely and defectiveness of clini- cal medication. Methods Unreasonable prescriptions of injection for intravenous drip was recorded and analyzed regularly by pharmacists. Results The unreasonable prescription included improper menstruum, drug concentration lack of standardiza- tion, drug compatibility, administration route, drugs using times, overdose and drug repeatability. Conclusion Pharmacists in PIVAS play an important role in practicing pharmaceutical care and improving the level of safe and avoiding defective drug use in the hospital.
