胃癌中β-catenin、ki67与Her-2/Neu的关系及临床意义

目的研究β-catenin、ki67及Her-2/Neu的蛋白表达与胃癌临床病理特征的关系,探讨分析β-catenin、ki67与Her-2/Neu蛋白表达、基因状态的相关性。方法采用免疫组化(SP)法检测101例胃癌标本中β-catenin、ki67及Her-2/Neu的蛋白表达,同时采用荧光原位杂交(FISH)技术对Her-2/Neu基因状态进行检测。结果β-catenin、ki67及Her-2/Neu的蛋白表达与胃癌临床病理特征关系密切:β-catenin、ki67与胃癌的分化程度、浸润深度、淋巴结转移具有显著相关性(P<0.05);ki67与胃癌TNM分期具有显著相关性(P<0.05);Her-2/Neu与胃癌的分化程度、TNM分期具有显著相关性(P<0.05),而与胃癌的浸润深度、淋巴结转移无显著相关性。ki67蛋白表达与Her-2/Neu蛋白表达及基因扩增状态具有显著相关性(r蛋白=0.567,P<0.05;r基因=0.304,P<0.05)。结论联合检测β-catenin、ki67及Her-2/Neu可作为了解胃癌生物学行为、胃癌诊断、判断预后的可靠指标。ki67可作为初筛Her-2/Neu基因状态的另一参考条件,对提高Her-2/Neu基因扩增检测效率、减少医用检测材料的浪费、指导临床靶向治疗可能有一定的意义。

Objective To study the relationship between the protein expression of β-catenin,ki67 and Her-2/Neu with the clinical pathologic characteristics of gastric cancer,and the correlation toβ-catenin and ki67 of the protein expression and gene conditions of Her-2/Neu. Methods Detected the protein expression of β-catenin,ki67 and Her-2/Neu by immunohistochemistry （ SP method） in 101 gastric cancer samples and the gene conditions of Her-2/Neu by fluorescence in situ hybridization （ FISH） . Results The protein expression of β-catenin,ki67 and Her-2/Neu had close relationship with the clinical pathologic characteristics of gastric cancer.β-catenin and ki67 had obvi-ous correlation to the differentiation, infiltration and lymphatic metastasis of the gastric cancer （P＆lt;0. 05）. ki67 had close relationship with the TNM staging of gastric cancer （ P＆lt;0 . 05 ） . Her-2/Neu had close relationship with the differentiation and TNM staging of gastric cancer （ P＆lt;0 . 05 ） , but had no relationship with the infiltration and lymphatic metastasis of the gastric cancer. The protein expression of ki67 had obvious positive correlation to the pro-tein expression and gene amplification conditions of Her-2/Neu （ rprotein =0.567, P ＆lt;0.05;rgene =0.304, P ＆lt;0.05）. Conclusion Association detection of β-catenin,ki67 and Her-2/Neu can be used as the reliable indexes in the process of understanding the biological behavior of gastric cancer,and gastric diagnosis and prognosis judge-ment and ki67 can be used as the reference in preliminary screening the gene amplification conditions of Her-2/Neu,which may have some significance in elevating the detection efficiency of Her-2/Neu gene amplification, re-ducing the detection material waste and guiding the clinical targeted therapy.
