清热活血法在痔病术后的用药规律

Medication regularity of prescriptions for heat-clearing and blood-activating method in hemorrhoids after surgery

目的:中药熏洗法在预防和治疗痔病术后并发症疗效显著,旨在分析探讨清热活血法在痔病术后中的用药规律.方法:检索中国知网(China National Knowledge Infrastructure,CNKI)、中国生物医学文献数据库(Chinese Biomedical Literature Database,C B M)等医学数据库中以清热活血法为主治疗痔病术后的临床研究报道(共检索出87篇符合标准的文献),收录方剂进行药物使用频率、归经和功效统计分析;并应用关联规则方法中的Apriori算法分析方剂中药对的配伍规律.结果:在87首方剂中,共出现了95味中药,其中出现两次以上中药共62味,出现频次共为691次.在药物核心系数(H)的统计中,苦参、五倍子、芒硝等药物的应用频次较多;在归经的统计中,归肝、胃、肺、脾经的药味和积分较高;在功效的统计中,以清热药、活血化瘀药、泻下药等使用次数较高;在药对的统计分析中,苦参-黄柏、苦参-当归、黄柏-地榆等药对的使用频率较高.结论:通过对药物核心系数(H)、归经、功效和药对的使用频率进行统计分析,能较好的发现应用清热活血法在痔病术后进行熏洗治疗的用药规律、配伍组成和临床疗效,为临床治疗痔病术后并发症用药提供理论指导.

AIM： To investigate the medication regularity of the prescriptions for heat-clearing and blood-activating method in hemorrhoids after surgery. METHODS： Clinical studies on heat-clearing and blood-activating method in hemorrhoids after surgery were searched from CNKI and CBM databases. A total of 87 papers meeting the criteria were retrieved. Data were extracted to analyze medication frequency, meridian affinity and efficacy, as well as herb-pairs principle using apriori method of connection rule. RESULTS： In 87 prescriptions, there were 95 Chinese medicines, of which 62 appeared twice. The appearing frequency was 691. The medicine core coefficients of Kushen, Wubeizi and Mangxiao were higher than those of others. With regard to herb meridian affinity, the integrals of liver meridian, stomach meridian, lung meridian and spleen meridian were higher than those of others. With regard to the herb efficacy, the use frequencies of heat-clearing herbs, blood-activating and stasis-dissolving herbs and purgatives herbs were high. With regard to herb pairs, the frequencies of Kushen-Huangbai, Kushen-Danggui and Huangbai-Diyu were high. CONCLUSION： Medication regularity of prescriptions for heat-clearing and blood-activating method in hemorrhoids after surgery was obtained by analyzing the core coefficient, meridian affinity and efficacy of herbs, which will provide a theoretical basis for clinical work.
