下鼻甲粘膜下部切除术治疗慢性肥厚性鼻炎30例

Prognosis of submucosal partial resection of the inferior turbinate mucosal resection for chronic hypertrophic rhinitis

目的观察下鼻甲粘膜下部切除术应用于慢性肥厚性鼻炎(chronic hypertrophic rhinitis,CHR)的治疗效果。方法选取2006年11月—2009年11月CHR患者50例,随机分为研究组30例与对照组20例。研究组采用下鼻甲粘膜下部切除术进行治疗。对照组采用激光方式治疗。对比分析两组术后6个月以及术后3年的手术效果。计量资料采用t检验,计数资料采用χ2检验,P<0.05为差异有统计学意义。结果术后6个月,研究组总有效率86.7%,对照组75.0%,两组比较差异无统计学意义(χ2=0.457,P>0.05)。术后3年,研究组总有效率80.0%,对照组45.0%,两组比较差异有统计学意义(χ2=6.551,P<0.05)。两组患者中均未出现感染、粘连以及狭窄等明显的并发症;但对照组患者中出现下鼻甲粘膜受损患者2例。结论下鼻甲粘膜下部切除术应用于CHR治疗预后效果更佳,值得临床推广应用。

Objective To explore the prognosis of submucosal partial resection of the inferior turbinate mucosa for chronic hypertrophic rhinitis（CHR）.Methods 50 patients with CHR from November 2006 to November 2009 were randomized into study group （n--30） and control group（n=20）.Study group underwent submucosal partial resection of the inferior turbinate mucosa,while control group received laser therapy.Both groups were compared for postoperative outcome 6 months and 3 years after surgery.T-test was used for measurement data, X2 test for count data, the result of P〈0.05 was considered statistically significant.Results Six months after surgery,the total effective rate was 86.7% in study group and 75.0% in the controls,which showed no significant difference （X2=0.457,P〉0.05）.Three years after surgery,the total effective rate was 80.0% in study group and 45.0% in the controls,which showed significant difference（X2=6.551,P〈0.05）.No infection,adhesion and stenosis occurred in both groups;however,two cases developed impaired inferior turbinate mucosa in control group.Conclusion Submucosal partial resection of the inferior turbinate mucosa for CHR shows a more favorable prognosis,which is worthy of clinical application.
