摘要
目的探讨术前超声心动图评价心脏黏液瘤及肺动脉压的诊断价值。方法回顾性分析86例患者的临床资料,根据肺动脉收缩压的高低将患者分为两组(高压组与正常组),分析两组患者的超声心动图对瘤体及其周边解剖关系的提示作用。结果高压组患者的黏液瘤总质量、过二尖瓣口流速、肺静脉口流速明显高于正常组,差异有统计学意义(P<0.05)。两组患者中,起源于左心房顶部的黏液瘤瘤蒂明显长于起源于其他部位的黏液瘤,差异均有统计学意义(P<0.05)。正常组患者起源于房间隔、左心房后壁的瘤体比其他部位起源的瘤体质量大,差异有统计学意义(P<0.05)。超声心动图的误判率为5.8%。结论原发性心脏黏液瘤的形态、大小、起源部位与肺动脉压有一定关系,超声心动图是术前诊断、评估的重要手段。
Objective To explore the diagnostic value of ultrasonic heartbeat graph to cardiac myxoma and pulmonary arterial pressure before operation. Methods Retrospectively analyzed the clinical data of 86 patients. They were divid- ed into two groups (high pressure group and the normal group) according to the pulmonary artery systolic pressure. In- vestigated the ultrasonic heartbeat graph that the anatomic relationship of the tumor and its surrounding. Results Myxo- ma total quality, mitral flow velocity,pulmonary vein flow velocity of high pressure group was obviously higher than the normal group, the difference was statistically significant (P〈0.05). Tumor originated from left atrial myxoma at the top of the origin was longer than that in other parts of the tumor, the differences were statistically significant (P〈0.05). The normal group were originated in theatrial septum, posterior wall of the left atrium tumor than in other parts of the tumor origin of mass,the difference was statistically significant (P〈0.05). The error rate of ultrasonic heartbeat graph was 5.8%. Conclusion Shape, size, site of origin primary cardiac myxoma of and pulmonary artery pressure have a certain relationship, ultrasound heartbeat graph is an important means for diagnosis, preoperative evaluation.
