原发性胆囊癌超声诊断评价及漏误诊原因分析

Evaluation on ultrasonic diagnosis of the primary gallbladder carcinoma and analysis on the causes of missed diagnosis and misdiagnosis

目的评价超声对原发性胆囊癌的诊断价值及漏误诊原因分析。方法选择胆囊占位病变98例,对其术前超声声像图表现进行分析,并与病理结果对照,重点分析超声诊断胆囊癌漏、误诊原因。结果经手术病理证实胆囊癌70例,超声诊断胆囊癌与病理结果对照符合率为75.7%(53/70);漏诊15例,漏诊率为21.4%(15/70),其中结节型5例(33.3%),厚壁型9例(60.0%),混合型1例(6.7%);误诊2例,误诊率为2.9%(2/70),皆为实块型。结论超声诊断胆囊癌有较高的准确率,超声诊断胆囊癌的主要易漏诊类型为结节型和厚壁型,易误诊类型为实块型。

Objective To evaluate the value of ultrasonic diagnosis of the primary gallbladder carcinoma and analyze the causes of missed diagnosis and misdiagnosis.Methods 98 cases with gallbladder occupying le-sions were collected to carry out a retrospective study and make a contrast of pathology results on the pre-operative sonogram and the causes of missed diagnosis and misdiagnosis were mainly analyzed.Results 70 cases of gallbladder carcinoma was confirmed by operation and pathology,accuracy：75.7%（53/70）;15 cases of missed diagnosis：21.4% （15/70）,and among them,5 cases of nodular type （33.3%）,9 cases of thick-wall type （60.0%）,1 cases of mixed type（6.7%）;2 cases of misdiagnosis：2.9%（2/70）,both solid biock.Conclusion The ultrasound diagnosis of gallbladder carcinoma had a higher accuracy.The main types of easily-missed diagnosis in ultrasound diagnosis gallbladder carcinoma were nodular type and thick-wall type,and the easily-misdiagnosed type is solid biock.
