自制单孔多通道腹腔镜穿刺装置在泌尿外科腹腔镜手术中的应用效果

Application effect of homemade single port transumbilical laparoscopic puncture device in urological laparoscopic surgery

目的探讨自制单孔多通道腹腔镜穿刺装置在泌尿外科腹腔镜手术中的应用效果。方法将87例拟行泌尿外科腹腔镜手术患者随机分为两组,观察组43例和对照组44例,观察组采用自制单孔多通道腹腔镜穿刺装置完成手术,对照组采用奥林巴斯推出单孔腹腔镜手术器械TriPort穿刺器完成手术,比较两组的手术费用、手术时间、并发症发生率及住院时间。结果观察组的手术费用明显低于对照组,差异有统计学意义(P<0.05)。两组的手术时间、并发症发生率和住院时间比较差异无统计学意义(P>0.05)。结论应用自制单孔多通道腹腔镜穿刺装置完成泌尿外科腹腔镜手术,效果等同于昂贵的单孔腹腔镜专用穿刺套管设备,该装置制造所用的原材料来源充足且价格低廉、制作方法简单,在很大程度上减少了手术费用,克服了单通道技术在临床应用的经费问题,为单通道腹腔镜技术的广泛开展奠定了基础。

Objective To explore application effect of homemade single port transumbilieal laparoscopic puncture device in urological laparoscopic surgery.Methods 87 cases of patients with laparoseopie operation in department of urol- ogy were randomly divided into two groups,43 cases in observation group and 44 cases in control group.Homemade sin- gle port transumbilieal laparoscopic puncture device was used in observation group,TriPort in laparoendoseopic single- site surgery from Olympus was used in control group.Operation cost,operation time,incidence rate of complication and hospitalization time in two groups was compared respectively.Results Operation cost in observation group was lower than that in control group,with statistical differenee（P〈0.O5）.Operation time,incidence rate of complication and hospital- ization time in two groups was compared respectively,with no statistical differenee（P〉0.05）.Conclusion Homemade sin- gle port transumbilical laparoseopic puncture device applied in urological laparoseopic surgery has the same effect with the expensive LESS,the raw materials are plentiful and cheap,manufacturing method is simple,operation cost are largely reduced,which has overcome financial issue of LESS in clinical application and laid a foundation for extensive develop- ment of LESS.
