锁定钢板与传统钢板治疗老年肱骨近端骨折的对比观察

Comparative observation of locking steel plate and traditional steel plate in the treatment of elderly patient with proximal humerus fracture

目的 比较锁定钢板与传统钢板治疗老年肱骨近端骨折的临床效果.方法 选取2009年3月～2012年2月本院收治的老年肱骨近端骨折患者120例,随机分为观察组和对照组,各60例.观察组采用肱骨近端锁定钢板治疗,对照组采用传统钢板治疗,比较两组的手术时间、术中出血量、住院时间、治疗费用及并发症发生率,并采用Neer评分评价两组患者的肩关节功能.结果 观察组的手术时间和住院时间短于对照组,术中出血量和治疗费用少于对照组,差异有统计学意义（P＜0.05）.观察组的疼痛评分低于对照组,功能、活动范围及解剖位置评分高于对照组,差异有统计学意义（P＜0.05）.观察组的并发症发生率为1.7％,对照组的并发症发生率为6.7％,差异有统计学意义（P＜0.05）.结论 与传统钢板相比,锁定钢板治疗老年肱骨近端骨折操作简单,内固定稳定,术中出血量少,更利于骨折愈合,值得临床推广应用.

Objective To compare the clinical efficacy of locking steel plate and traditional steel plate in the treatment of elderly patient with proximal humerus fracture.Methods 120 elderly patients with proximal humerus fracture in our hospital from March 2009 to February 2012 were selected and randomly assigned to observation group and control group,60 eases of each group.Proximal humerus locking steel plate was used in observation group,traditional steel plate was used in control group. Operation time,the amount of bleeding,hospitalization time,treatment cost and incidence rate of complication was compared in two groups respectively.Shoulder joint function of patients in two groups was evaluated according to Neer score.Results Operation time and hospitalization time in observation group was shorter than that in control group respectively,the amount of bleeding and treatment cost in observation group was less than that in control group respectively,with statistical difference （P〈0.05）.Pain score in observation group was lower than that in control group,function score,range score and anatomical position score in observation group was higher than that in control group respectively,with statistical difference（P〈0.05）.fneidence rate of complication in observation group was 1.7%,inci- dence rate of complication in control group was 6.7%,with statistical differenee（P〈0.05）.Conclusion Compared with traditional steel plate,locking steel plate demonstrates simple operation,stable internal fixation,less intraoperative blood loss and facilitates healing of fracture in the treatment elderly patients with proximal humerus fracture,which is worthy of clinical promotion and application.
