北京市德胜社区签约慢性病患者慢性病防治知信行现状调查

Analysis on chronic disease related Knowledge-Attitude-Practice( KAP) situation among patients in Desheng Community of Beijing

目的了解北京市德胜社区家庭医生式服务开展初期,签约慢性病患者慢性病防治知信行现状,为有效推进家庭医生式服务,落实健康管理提供依据和参考。方法采取分层随机抽样方法,以中心门诊7个"家医"团队中所需签约家庭总数(4200户)的10%为样本量,利用自行设计的问卷对2012年6—10月来中心门诊就医的420名新签约慢性病患者进行调查。结果 "家医"签约初期,签约慢性病患者慢性病防治知信行总体得分为(76.47±9.76)分,其中知识部分为(8.44±2.95)分,态度部分为(7.96±1.77)分,行为部分为(60.07±8.51)分,均在中位数以上;人群总分及格率为67.14%、达标率为0.71%,其中知识部分及格率为20.48%、达标率为1.67%,态度部分及格率为59.05%、达标率为21.19%,行为部分及格率为73.10%、达标率为4.76%。结论 "家医"签约初期,签约慢性病患者慢性病防治知信行总体得分超及格水平,离达标要求仍有一定距离。国家推行"家医"服务,落实家庭健康职责势在必行。

Objective To understand chronic disease related Knowledge-Attitude-Practice （KAP） situation among patients who had signed with their family doctors at the initial stage of family doctor service system in Desheng Commu- nity of Beijing, and provide reference for carrying forward family doctor service system and health management. Methods To use stratified random sampling method, 420 chronic disease patients （10% of 4200 families） who signed with 7 family doctor service teams in center outpatient service of Desheng Community were selected, and a self-designed questionnaire was used to investigate their KAP status, which came to center outpatient service from June to October, 2012. Results At the initial stage of family doctor service system, the overall score of chronic disease KAP among the patients was （76. 47 ± 9. 76）. The scores of knowledge, attitudes and performance were （ 8.44 ± 2. 95 ）, （7.96 ± 1.77 ） and （ 60. 07 ±8.51 ）, respectively. All the 4 scores were above their median. The overall KAP pass rate and satisfied rate were 67. 14% and 0. 71%, and 20.48% and I. 67% for the knowledge, 59. 05% and 21.19% for the attitudes, and 73.10% and 4. 76% for the performances, respectively. Conclusion At the initial stage of family doctor service system, the chronic disease related KAP overall score of patients was above the pass score, there still had low satisfied rate. Therefore, implementation of family health responsibility is imperative for carrying forward family doctor service system.
