显微镜下精索静脉曲张单纯静脉结扎术疗效分析(附66例报告)

Curative effects analysis of spermatic vein ligation under microscope for varicocele treatment

目的探讨显微镜下单纯静脉结扎术治疗精索静脉曲张的安全性及临床疗效。方法回顾性分析2012年7月至2013年5月采用显微镜下静脉结扎术治疗精索静脉曲张的66例患者的临床资料，就手术时间、精液质量、并发症等进行3个月以上随访。结果所有病例手术均成功，手术时间平均97分钟，其中单侧的42例平均手术时间为87分钟，双侧的24例平均手术时间为113分钟。精液质量有不同程度改善。术后住院2—3d，平均2．6d，均未发生阴囊血肿；6例术后阴囊水肿1个月内均消失。术后复查阴囊彩超所有患者均未出现睾丸萎缩，随访3～12个月均未见复发。结论显微镜下单纯精索静脉结扎术治疗静脉曲张具有手术操作简单、疗效好，安全高、创伤小、并发症少，术后恢复快等优点，且更经济，便于基层推广，具有较高的临床应用价值。

Objective To investigate the safety and clinical efficacy of the spermatic vein ligation under the microscope for varicocele treatment. Methods Retrospective analysis the clinical data of 66 varicocele patients who received vein ligation under in our hospital from July 2012 to May 2013, and follow-up for collecting the data of the operative time, semen quality and complications. Results All the operations were completed successfully, mean operative time was 97 minutes, mean operation time of patients （n = 42） received unilateral ligation was 87 minutes, and 113 minutes of bilateral （n = 24）. Semen quality improved in varying degrees. Postoperative hospital stay was 2 -3d, average 2.6d, all cases did not suffered from scrotal hematoma; 6 patients suffered from scrotal edema and disappeared within a month. All the patients were rechecked by Scrotal B- ultrasonic after operation, and none of them appeared testicular atrophy. None patients recurrence within 3 - 12 months. Conclusions Spermatic vein ligation under microscopic was a operation which had a high clinical value for grassroots marketing because of its better effect, higher security, smaller trauma, fewer complications, faster recovery and more economical.
