双能量技术检测痛风患者尿酸盐沉积 被引量:12

摘要 痛风是由尿酸钠结晶沉积所致的相关性关节病,与嘌呤代谢紊乱和尿酸排泄减少所致的高尿酸血症直接相关。急性特征性关节炎主要包括急性发作性关节炎、尿酸盐沉积形成;慢性尿酸盐沉积疾病包括尿酸盐沉积性慢性关节炎、尿酸盐沉积肾病和尿酸性尿路结石,重者可出现关节残疾和肾功能不全。近年来痛风的发病率呈上升趋势[1]。目前临床上诊断痛风依靠传统影像学检查方法,然而这些影像学方法不能明确标识痛风石。由于尿酸盐结晶在不同能量下存在不同衰减,所以采用双能量技术可检测出体内沉积的尿酸盐结晶。双源CT ( dual-source CT,DECT)将尿酸盐沉积彩色编码为红色,钙化为蓝色,所以,当实际应用到泌尿系结石患者时, DECT可以高度精确地诊断尿酸性肾结石,并将其与体内和体外的含钙结石区分开[2]。该研究中,作者利用DECT检测痛风患者四肢关节的尿酸盐沉积及量化尿酸盐沉积体积,报道如下。
