微创钻孔引流术联合尿激酶灌注治疗高血压脑出血的体会 被引量:4

摘要 目的 探讨微创钻孔引流术联合尿激酶灌注治疗高血压脑出血的疗效.方法 术前经头颅CT定位,明确穿刺进针点,选择血肿最大直径方向穿刺,注意避开血管和功能区.首次抽吸出血肿总量的65%~75%为佳,合并破入脑室者行侧脑室引流术效果更好.复查头颅CT后,经三通向引流管缓慢注入尿激酶达血肿腔溶解血肿,引流2~5 d,最长不超过1周,血肿消除85%以上或残留血肿<10 mL即可拔引流管.结果 本组63例患者,其中存活60例,占95.2%,随访1~3个月,ADL(日常生活能力) Ⅰ级12例(19.0%),Ⅱ级21例(33.3%),Ⅲ级16例(25.4%),Ⅳ级9例(14.3%),V级2例(3.2%).结论 高血压脑出血行微创钻孔引流术联合尿激酶溶解血肿治疗,具有不受技术条件限制、损伤较小、操作简单、手术时间短、有利于血肿引流、缩短病程、住院时间短、费用低、术后并发症少的优点.
